Our chaplains, social workers, and hospice volunteers are dedicated to supporting you through bereavement, serving as guides and companions. They are here to listen, assist with communicating feelings and final wishes, and help arrange memorials or clarify important final thoughts. This is our privilege and we are pleased to serve you.

Services We Offer

At Health Care Connectors, our goal is to act as resource that provides support and guidance for families and loved ones experiencing grief and loss. These services include:

  • Informational and educational resources
  • Referrals to appropriate therapeutic and community resources
  • Spiritual care to help patients and families navigate questions of personal meaning, purpose, hope, and faith during challenging times
  • Guidance from professionally trained chaplains, offering sensitive and inclusive support for individuals of all faiths and beliefs

We provide our services at no cost to the families and friends of our hospice patients for at least 13 months after their loved one’s passing, ensuring continued support during the journey of healing. While bereavement care offers support through the grieving process, spiritual care provides guidance and reassurance for individuals seeking meaning, peace, and connection during life’s transitions.
