Physical Therapy

Concierge physical therapy is private pay, in-home physical therapy scheduled around your family’s calendar. There is no homebound requirement and no set certification period, so care continues for as long as it’s helping, at the client’s home or their assisted living community. Health Care Connectors provides concierge physical therapy throughout Fairfield County, including Greenwich, Stamford, Darien, New Canaan, Westport, Norwalk, Wilton, Weston, Ridgefield, Fairfield, Trumbull, and Bridgeport.

Physical Therapy 500x408

What Happens During a Visit

Your therapist begins with a full evaluation of strength, balance, gait, flexibility, and pain, along with a review of your medical history and recovery goals. From there, each visit includes hands-on treatment and guided therapeutic exercise, a home exercise program to continue between sessions, and education for family members or caregivers involved in daily support. Your plan is reviewed and adjusted at each visit as you progress.

Working Toward Your Goals

Clients use concierge physical therapy to work toward the mobility, strength, balance, and recovery goals that matter most to them, on their own timeline. Common goals include walking safely without a cane or walker, preparing for knee, hip, or shoulder replacement surgery with pre-surgical strengthening and mobility work, rebuilding strength and endurance after a hospital stay or period of inactivity, recovering from elective or cosmetic surgery, managing conditions such as Parkinson’s disease, multiple sclerosis, arthritis, or vertigo, and reducing fall risk while improving balance, mobility, and confidence moving safely through the home.

Frequently Asked Questions

Do I need a physician referral for concierge physical therapy?

No referral is required. Sessions can begin as soon as you’re ready.

Where do visits take place?

At the client’s home or their assisted living community.

How often can I schedule sessions?

As often as helpful. Sessions are scheduled directly with your family rather than set by a fixed program, and can continue for as long as they’re benefiting you.

Is concierge physical therapy covered by insurance?

It’s a private pay service, billed directly rather than through insurance.

Can concierge physical therapy help me prepare for surgery?

Yes. Many clients use it in the weeks before a joint replacement or other procedure to build strength, practice bed mobility, and learn what to expect during recovery.
