When a patient is recognized as eligible for hospice by their physician, the transition can begin as soon as they, or their designated healthcare decision-maker, agree to shift from a curative focus to embracing the compassionate comfort-based care offered by hospice services.
Our Care Plans
Our team includes doctors, licensed nurses, social workers, chaplains, and others who ensure our patients have every needed resource. We serve our patients by putting good, caring people by their side, whether they live in a house, apartment, or an assisted living community. Some patients reside in a 24/7 skilled nursing facility, and our hospice services complement the care they receive from on-site nurses.
Hospice Care Services
Deciding to go into hospice care is a personal and sensitive choice that is made with the help of a physician. It’s best considered if your loved one has a life expectancy of six months or less; this is a time when the focus shifts from active treatment to ensuring comfort by managing their pain and enhancing their quality of life. Give us a call, and a representative can explain the full range of care we offer. A nurse will evaluate, and a personalized care plan will be developed with the patient’s goals as the priority.
We provide compassionate support for our patient’s medical, emotional, and spiritual needs, as well as assisting with daily self-care tasks like incontinence care, bathing and dressing.

Grief may manifest in many phases and stages for you and your loved one. Our bereavement services are designed to address a range of practical, emotional, and spiritual needs, providing support throughout this journey.
When a Medicare patient chooses hospice care, the Centers for Medicare and Medicaid Services (CMS) will directly cover the costs related to the terminal diagnosis through the hospice provider. Any physician services needed are billed by the hospice based on the type of service provided.

Our mission is to make comfort medications accessible and delivered to you when and where you need them 24 hours per day.
Access to Doctors/Nurses
Your nurse makes regular visits to assess you and provide additional care or other services. The nurse, along with the Hospice doctor, are on-call 24 hours a day, seven days a week to meet your needs.
