Speech Therapy

Medicare-certified speech therapy is in-home speech therapy ordered by a physician as part of a home health plan of care, covered under Medicare Part A at no cost to eligible patients. It follows a clinical plan built around specific recovery goals for communication, swallowing, and thinking skills, most often after a stroke, illness, surgery, or hospital stay. Health Care Connectors provides Medicare-certified speech therapy throughout Fairfield County, including Greenwich, Stamford, Darien, New Canaan, Westport, Norwalk, Wilton, Weston, Ridgefield, Fairfield, Trumbull, and Bridgeport.

What Happens During Your First Visit

Your speech-language pathologist completes an evaluation of speech, language, voice, swallowing, and thinking skills, then sets recovery goals with you and communicates with your physician to build an individualized treatment plan. Every visit after that works toward the goals in that plan, with progress shared with your physician along the way.

Under a physician’s plan of care, our speech-language pathologists treat patients recovering from or managing:

Stroke and acquired brain injury
Swallowing difficulties (dysphagia)
Parkinson’s disease and multiple sclerosis
Memory and cognitive changes affecting daily communication
Voice weakness following illness or extended hospitalization
Communication changes following surgery or a hospital stay

Do I Qualify?

Medicare covers speech therapy at home when a physician orders it as part of a home health plan of care, the patient is homebound, there is a skilled therapy need, and the patient is under a physician’s care. Homebound doesn’t mean a patient can never leave home. It means leaving takes real effort, often requiring help from another person or a device like a wheelchair or walker. Patients may still leave for medical appointments and occasional non-medical outings while qualifying.

When Medicare Visits End

Once the goals in your plan of care are met, Medicare-certified visits conclude. If continued therapy would still be helpful at that point, our team can help your family arrange ongoing support so there’s no gap in care.
