Home Health Care Services

We provide care through one-hour home visits by specialized clinicians, continuing until recovery goals are achieved. Episodes of care typically last between 30 and 60 days but can be renewed if medically necessary. Home health care visits are generally covered 100% by Medicare through the Medicare home health benefit or by private insurance.

With our skilled nursing care, a licensed nurse will work with you or your loved one’s physician and visit the home to provide high-level medical care.

Our physical therapy services can help improve strength and balance needed to increase safety with walking and to reduce the risk of falls. It can also help reduce pain and increasing overall mobility.

When daily living skills (such as getting dressed or taking a shower) become a problem, occupational therapists can help rehabilitate weak areas and teach compensatory strategies so patients can continue to enjoy everyday life.

We have skilled speech therapists available to address your loved one’s communication issues. They can work with them, their caregivers, and physicians to help improve not only their speech but also swallowing, memory and problem solving skills, organization, and more.

Home health aides provided by Medicare can help with daily self-care tasks like incontinence care, bathing, dressing, and other needs.

Medical social workers connect you and your caregivers with the community resources you need.

Remote Patient Monitoring, or RPM, is the use of digital devices that allow your physician to monitor your condition remotely.
