Grief may manifest in many phases and stages for both you and your loved one. Our bereavement services are designed to address a range of practical, emotional and spiritual needs, providing support throughout this journey.
Your loved one needs more than just medical attention; their emotional and spiritual needs are equally important. Our chaplains, social workers, and hospice volunteers are dedicated to supporting you through bereavement, serving as guides and companions. They are here to listen, assist with communicating feelings and final wishes, and help arrange memorials or clarify important final thoughts. This is our privilege and we are pleased to serve you.
At Health Care Connectors, our goal is to act as resource that provides support and guidance for families and loved ones experiencing grief and loss. These services include:
We provide our services at no cost to the families and friends of our hospice patients for at least 13 months after their loved one’s passing, ensuring continued support during the journey of healing. While bereavement care offers support through the grieving process, spiritual care provides guidance and reassurance for individuals seeking meaning, peace, and connection during life’s transitions.
We provide compassionate support for our patient’s medical, emotional, and spiritual needs, as well as assisting with daily self-care tasks like incontinence care, bathing and dressing.
Care. Compassion. Empathy. You can count on your hospice nurses to ensure your loved one’s comfort through expert care and medication management.
Your loved one should not face end-of-life with unmanaged pain. Pain management will improve the quality of life for patients by allowing them to engage more fully and cherish meaningful moments with loved ones.
Music therapy, massage therapy, and other therapeutic modalities are integral parts of the care we offer in hospice. Holistic care is a vital part of what we do.
Special medical equipment can greatly enhance your loved one’s comfort at this time. We provide everything needed to ensure their well-being and your peace of mind.
Our hospice experts are ready and waiting for your call. We’ll ensure you get the help you need, day or night.
When a Medicare patient chooses hospice care, the Centers for Medicare and Medicaid Services (CMS) will directly cover the costs related to the terminal diagnosis through the hospice provider. Any physician services needed are billed by the hospice based on the type of service provided.
