The interdisciplinary team usually consists of:
- The patient’s personal physician
- Hospice physician (or medical director)
- Nurses
- Hospice aides
- Social workers
- Spiritual care providers or other counselors
- Bereavement professionals
- Speech, physical, and/or other occupational therapists
- Trained volunteers
