A Personal Approach to Home Health Care in Fairfield County

This group of care givers is outstanding. When my mom was released home from rehab she had a hard time with the adjustment. Thankfully, this team has been there for her. I would like to particularly recognize Nurse Olivia for her compassion and care while still maintaining her professionalism. She has been communicative every step of the way [which is key in my book]. She is definitely in the right line of work to make sure that her clients and familes of her clients are all on the same page with the same goal of a dignified and safe return to home after rehab. Thank you team! You are terrific!

I have had the pleasure of having Diana and Olivia as my nurses for the past few weeks. They have been wonderful and caring. They answer my questions and never make me feel rushed.

HCC has been working with my elderly mother for quite sometime now. We have had a few different nurses and aides and every single one of them has been awesome!
Especially; Diana, RN Aisha, Aide and Ashley, PT who were absolutely professional, kind, courteous and communicate well with the patient, family and the doctor. If you want your loved ones taken care of, go with HCC.

Hello, I just had my first visit with Eddie the nurse, which was absolutely superb. He was totally understanding very adapt at what he was doing and it was a very successful visit. I am very happy that Eddie will be my nurse in the future and I look forward to seeing him next week thank you very much. Health Care Connectors has done a phenomenal job. They are incredibly supportive while caring for our in home health care needs. They have a great staff that works together as a professional team. HCC provides a variety of services that makes it extremely easy to care for our family needs. Edith Giron is an absolute angel who comes every day to assist us. Her patience and attentiveness is the perfect fit for us. Yaw is also a true asset to the team. He frequently visits to ensure all things are running well and provides extremely valuable advice. His mentorship and dedication to the HCC team and their clients is priceless. Yaw makes everything extremely easy. He clearly trains and mentors the HCC team in a manner that makes his team among the very. Est in the business. I honestly can’t say enough about the professionalism and service provided by HCC. I highly recommend them. Finally, I can’t say enough about Edith Giron who has been with us for nearly two years. She is truly special and an asset. It’s as if she is family now.

We were able to get palliative care for a month which really helped mom succeed back to eating and communicating a little bit better. I know the visiting nurse, the OTs and PTs Successfully worked with her.

Everyone has been professional personable and so very friendly as we undergo our next chapter with our mother. I cannot say enough, kind words and appreciation for Health Care Connectors. Again, thank you so much for all that you’ve done and will continue to do until the end of May.

After an unexpected emergency surgery, we needed home therapy and nursing services. I prayed about finding the right fit close to home. My Google search led me to HCC. The website pulled me in… seemed genuine. Then the hospital gave me three options and HCC was one of them. That did it for me.
And it has been a rewarding decision. No regrets.

They were available one day after discharge following major disabling Orthopedic Surgical repair of my Quadriceps tendon. I was visited by Skilled nurse, occupational therapist, and Physical Therapist to assess my immediate needs as well as plans for beginning the needed Physical Therapy which could only start at four weeks. This would be in my home before I could even walk or be transported to an out patient facility.
From the very first visit by the home care Therapist I was unburdened of my concerns that I might be immobilized forever. A Patient thorough examination with skilled knowledge of my specific limitations, and coordinated with my surgeon’s orders began a gentle process of retraining injured muscle, and repaired tendon in a safe progressive order leading me to weight bearing, cane assisted walking, stair climbing and knee flexion enabling driving myself with renewed independence. My therapist was proficient, gentle, and skilled, always supportive of my flagging energies. It was her experienced manner, and kindness that brought me thru the first six weeks of home bound recovery to emerge on my feet to progress to the next months of outpatient rehabilitation.
I could not have achieved this healing state without the home care physical therapy I received!

Very pleased with the services provided by Healthcare Connectors. Gretchen Singsen OT is an outstanding and dedicated therapist who has worked with my elderly father after he broke his wrist. She goes the extra mile to connect with her patients and to identify their specific needs, even in complex cases. We are grateful for her hard work and excellent bedside manner.

It is with great pleasure that I am writing this review for Home Care Connectors of Greenwich CT. They were able to provide an exceptional CNA , Sakhile , the day after I called them. I had major back surgery and needed alot of help with daily tasks. I had an unpleasant experience with my previous caregiver from another agency so I was reluctant to try again. I couldn’t have been more pleasantly surprised.
Sakhile , my CNA, is one of the brightest, kindest , gentlest humans whom I have ever had the privilege of meeting. He is always at least one step ahead of me ,offering exactly the right amount of care and support which is ever changing. He cheers me on and encourages me. He lets me think that I’m doing it on my own even though he’s constantly paying close attention to what I’m doing and is always there if I stumble . He has a great sense of humor and is a brilliant chess player.
He has helped me through a difficult time physically and emotionally as I very recently had to put my 92 year old mom into an assisted living facility. He has become a dear and trusted friend who has brought me much joy.

Highly recommend Health Care Connectors in Greenwich! My home nurse was Gladys, who provided me with the physical and mental care I very much needed after my surgery. Our connection was immediate. Her genuine warmth and caring manner put me at ease as it was easy to see that she was there to give her all to make me comfortable. With her excellent professional experience and background knowledge, she was able to answer all my questions. Her added knowledge of wound care gave me the comfort and reassurance that all was going well before my post op doctors appointment.
Gladys has the unique ability to problem solve and assess a situation before making a decision. A characteristic I found very helpful. She very patiently listens, giving value and support to my pain and then offering information that proved very helpful.
I cannot say enough positive about her care. She was exactly the person I needed to help me. I am extremely thankful to her and commend your agency for hiring such an exceptional individual!

It is with pleasure that I recommend the services of Home Care Connectors and particularly Tutu Olomela, administrator to assist with in-home care. Our specific needs of 24 hour care included transport, caregivers, nursing and physical therapy. Tutu was there to respond immediately, offering flexibility of services in a kind and considerate manor. Financial accounting was accurate, thorough and dependable. And most important, the caregivers were competent, caring and professional.

The HomeCare Connectors team is amazing! JP is a fountain of knowledge and advice on navigating what is a really tough time, and one that most of us (luckily) don’t have to go through often. He and the team were always available to answer questions and went above and beyond in caring for my dad’s partner. We build a strong relationship and I’m happy to have JP as a resource as I now navigate my dad’s long term care situation. I highly recommend HCC!

Upon being released from the Civita Care on Long Ridge Road I was sent to Healthcare Connecters of Greenwich.for a follow up. Since I had a rocky start with them before I went to the convalescent hospital I was cautious.
My time this time was one million percent better. The nurse, physical therapy, and the aides the have sent are punctual and very nice.

I have ALS. I need help to do the big and little things in life , like dressing, eating, communicating, and breathing. My aides need to be intuitive, caring, skilled, and patient. Every day. All the time. Those are non negotiables.

When we partnered with JP and his company, Home Care Connectors, for my care, we received all of these necessary traits and so much more. JP’s initial conversation with our family led to a true partner in my first aide. JP placed Yaw with our family, and we are forever grateful . Yaw was not only impressive in his skill and professionalism, he was insatiably curious – learning with us as my needs progressed. Yaw, like JP, made me feel truly seen and heard. He became part of our family.
Yaw has since moved into HCC’s leadership team, ably helping to manage and mentor the agency’s aides in the field. While we miss seeing Yaw every day , he is a regular visitor – a resource for my current aides and a treasured friend to our family.

That level of care and skill has not wavered with my current lead aide, Nabila . Nabila has spent over a year with our family, navigating my needs, including a few scares, with calm, confidence, and no small measure of humor.

I am still working full time, and that would not be possible without aides comfortable in a highly interactive professional setting . I am a school administrator, and my days routinely involve traveling to and around campus, visiting classes, meeting with colleagues, and working with families . To do my job well , I need a partner in an aide who is not only comfortable in these settings, but truly enjoys it all. JP heard this need in our first meeting, and Yaw and Nabila have delivered.

Home Care Connectors is the ultimate in professional, caring, and coordinated assistance in the home. Whether it be post-surgery, chemotherapy, or aging in place, this is the best available. Starting with the owner, JP sets the tone and high standard of excellence, caring, competence, and attention to detail. All the aides that I have had were exceptional, different in many ways, but each capable and caring.
My favorite aide is Monica F., who loves to cook, and since I love to eat, it was a great match. Her positivity and sunny outlook were contagious, as is her energy. My sister and I were looking for in-home companion care for our 90 year-old parents who still live independently but needed help getting to the grocery store, doctor appointments, and more. My sister Donna lives an hour from our parents and works full-time for a busy medical office. Being available and ‘on-call’ for our parents was becoming more difficult. Donna wanted to help, but wasn’t always able to do so. She felt guilty and increasingly concerned for our parents’ safety and well-being. I felt helpless because I live 1,800 miles away. I wanted to make things better for my parents AND my sister.

We took the time to interview several home care agencies in Fairfield County and like most adult children with aging parents, we wanted to be sure mom and dad would be well taken care of and in the hands of caring, compassionate, and trustworthy people.

JP and Heather drove nearly an hour one-way (typical I-95 traffic!) to meet my sister, my parents and me for the first time at our parents’ home. They explained companion care to mom and dad who were skeptical at the thought of allowing aides (total strangers) into their home. By the end of their visit, JP and Heather made us all feel well-informed and at ease scheduling the first companion appointment for our parents.

When you speak with JP, you feel like you’re talking with a member of your family who cares about your parents’ health and well-being the same way you do. He called or emailed my sister and me regularly to ensure we were feeling good about the level and quality of care mom and dad were receiving. What business owner personally extends that level of customer service these days?!?

JP’s compassion and empathy are true and transparent. His team helps take the worry and weight off your shoulders as you navigate this unfamiliar and emotional process. I want to share my sincere thanks for the outstanding job that Home Care Connectors has done for our family. Having just moved my mother and father (both in their mid-80’s) to an assisted living facility, we found ourselves in need of additional services. Home Care Connectors was right there, willing to step in with an hours’ notice. The owner of the company JP Muir and Heather Flaherty, RN, were at my parent’s apartment to perform an assessment in an hour; that is unreal customer service. Upon reviewing their needs, they selected Fleurise to provide them with home care assistance. Fleurise is a true professional and knows how to handle her elderly clients.
My father passed shortly after, and Home Care Connectors has been there, providing grief support, elderly aide, and shoulder to lean on for my mother.
